Job Description Job Description Home Health RN Clinical Manager Location : Hanover, PA Department : Home Health Reports To : Director of Clinical Services Position Type : Full-Time, Exempt, Monday-Friday, 8:00 am-4:30 pm Summary : We are seeking an experienced and dedicated Home Health Clinical Manager to lead our interdisciplinary team, including RNs and LPNs. The Clinical Manager will ensure the delivery of high-quality care while fostering staff development and operational efficiency. This position requires a compassionate leader committed to upholding the organization’s mission, vision, and values. Key Responsibilities : Patient Care Coordination Oversee scheduling and ensure adherence to individualized care plans. Conduct team meetings and patient care conferences. Collaborate with patients, families, physicians, and care teams for optimal outcomes. Personnel Management Supervise and evaluate clinical staff performance. Assist in the recruitment, onboarding, and orientation of new team members. Conduct performance evaluations and staff development initiatives. Operational Oversight Manage caseloads and productivity. Ensure compliance with regulatory standards. Participate in administrative on-call rotations and insurance authorizations. Leadership Promote a collaborative work environment. Facilitate professional growth and team cohesion. Act as a resource for clinical problem-solving and decision-making. Performance Improvement Work with Quality Improvement to enhance patient outcomes. Ensure compliance with Medicare and PA Home Health Licensure standards. Customer Service Foster positive relationships with internal and external stakeholders. Maintain accountability for patient and staff satisfaction. Qualifications : Current PA RN license, BSN preferred. Minimum 3 years of clinical experience in Home Health. Strong leadership skills Valid driver’s license with reliable transportation. CPR certification. Strong leadership, communication, and organizational skills.
