Position Overview Join our team as a Universal Care Manager within our Community First Choice (CFC) Program, a role pivotal in shaping the lives of those we serve. As a beacon of compassion and dedication, you will play an integral part in coordinating and managing personalized services, ensuring our clients lead independent and dignified lives in their communities’ comfort. Key Responsibilities – Perform detailed assessments to evaluate the needs and preferences of our participants. – Craft and execute tailored care plans in collaboration with participants, their families, and other caregivers. – Oversee the provision of services, guaranteeing alignment with the goals and needs of our clients. – Offer continuous support and advocacy for our participants, proactively addressing concerns. – Keep meticulous records of all services and interactions, ensuring information is current and accurate. – Work in partnership with community resources and service providers to fortify the support network for our clients. – Engage in ongoing educational and professional development opportunities to remain at the forefront of care management practices. Qualifications – Bachelor’s degree in Social Work, Nursing, or a related discipline. – A minimum of 2 years experience in care management or a similar role. – Deep understanding of the unique needs and challenges confronting older adults and individuals with disabilities. – Exceptional skills in communication, organization, and problem-solving. – Self-motivated with the ability to excel independently and as a team member. – Competency with electronic health records and relevant technological tools.
