Job Description Job Description Adult Transition Manager Center for Independent Living of South Florida | www.cilsf.org Full-time with benefits Hybrid schedule, Mondays through Fridays, 8:30 AM to 5:00 PM $50,000.00 annually Responsibilities Under the general supervision of the management team, the Adult Transition Manager following the Independent Living (IL) philosophy will offer a variety of customized services, workshops and training to program participants and people with disabilities, with the intent of supporting their educational and employment goals and providing them with the necessary tools to live independently. The Adult Transition Manager supervises the Adult Transition Advocate in addition to contracted employment specialists. Oversee the intake process of program participants, assessment of their needs and development of their Independent Living Plan that supports their goals. Monitor participants’ progress towards their goals and improve processes and services where needed. Observe participants’ class interaction and assess need for accommodations. Coordinate training and other educational opportunities as needed. Maintain progress notes and service records ensuring they’re in compliance with program and grant standards and readily available upon request. Ensure that annual participant and stakeholder satisfaction surveys are completed and delivered on time. Compile performance statistics on a monthly basis and ensure reports to funders are accurate and timely submitted. Work closely with Accounting to ensure invoices are also processed accurately and on time. Develop and maintain partnerships with local school staff including Counselors, Division of Vocational Rehabilitation Counselors, among other community service providers. Work closely with other community agencies to provide program participants with additional services which CILSF does not provide. Participate and oversee the participation of program staff in community activities, such as job fairs, parent meetings, and other community events that support the program and participants’ goals. Advocate for and assist participants in obtaining benefits, such as Social Security Disability Insurance, Supplemental Security Income, Medicaid, Medicare, food stamps and other county, state and federal benefits. Be familiar with the regulations of local, state and federal programs, among other providers, and be able to offer advocacy advice to participants and their families. Provide peer support and peer counseling as participants work towards their goals. Maintain communication with participants’ parents or caregivers as appropriate to ensure quality study time and home environment. Represent the organization in various outreach settings to educate the broader community about the organization’s services and rights of people with disabilities. Disseminate and promote the philosophy of Independent Living, self determination and self advocacy. Proactively research and assist management in applying for potential funding opportunities to enhance and expand both the program and organization. Establish with management appropriate measurements of quality improvement and help evaluate the performance of program staff on an ongoing basis. Participate in training, workshops and other activities for professional development. All other duties as assigned. Qualifications Must possess a Bachelor’s degree or higher in Social Work or related field. Two years in a nonprofit employment service agency with experience in job placement, coaching and counseling for individuals with disabilities preferred. Experience in the social services delivery system in Miami-Dade County preferred. Must possess knowledge of the Americans with Disabilities Act of 1990 and other disability rights movement milestones; plus knowledge of basic Social Security, housing, transportation, medical and other benefits. Bilingual preferred (Spanish or Creole). Excellent writing and communication skills in addition to public speaking skills required. Experience working with individuals from different cultures and with various disabilities. Must have access to reliable transportation.
